The 210th Brigade Support Battalion was first constituted as the 10th Medical Battalion on 27 August 1942. On 9 September 1942 the 10th Medical Battalion was activated at Camp Carson, Colorado. Major (Doctor) Harry L. Berman, the first commander assumed command on 10 October 1942.

On 15 July 1943 the 10th Medical Battalion was reorganized and assigned to the 10th mountain Division. During World War II the 10th Medical Battalion provided medical support for the 10th Mountain Division as it fought along the length of the Italian Peninsula. For its actions the 10th Medical Battalion was awarded battle streamers for the North Apennines and Po Valley campaigns. Following the end of World War II the 10th Mountain Battalion was inactivated at Camp Carson, Colorado.

On 1 July 1985 the 10th Medical Battalion was reactivated and again assigned to the 10th Mountain Division (Light) at Fort Drum, New York. On 1 October 1990 the 10th Medical Battalion was reorganized and redesignated as the 210th Forward Support Battalion. This multifunctional battalion provided supply (HQ/A Company), maintenance (B Company), and medical support (C Company) to the 2nd Brigade Combat Team of the 10th Mountain Infantry Division (Light), as a part of the 10th Mountain Division Support Command (DISCOM). On 24 August 1999, the battalion began to deploy to Bosnia-Herzegovina in support of Stabilization Force 6 (SFOR6) where it became the 210th Logistics Task Force. The battalion redeployed in March 2000 and transitioned back to the 210th Forward Support Battalion.

During Operation Iraqi Freedom, the 210th Brigade Support Battalion operated around the Baghdad area in 2004. Following this deployment the 210th FSB returned to Fort Drum and was inactivated as part of the transition of the entire 10th Mountain Division to the US Army's new modular force structure. As a result the Division Support Command (DISCOM) was inactivated, its 3 Forward Support and 1 Main Support Battalions forming Brigade Support Battalions for the Division's 4 reorganized Brigade Combat Teams. The 210th Forward Support Battalion was reactivated and redesignated as the 210th Brigade Support Battalion. The reorganized unit was made up of four integral companies (HHC, Supply and Distribution, Maintenance, and Medical), along with Forward Support Companies attached the four combat battalions of the 2nd Brigade Combat Team.
