CV-22 Accident Investigation Board Complete

Air Force Special Operations Command

Release Number: 120005

8/30/2012 - HURLBURT FIELD, FLA. -- Air Force Special Operations Command officials have released the results of their investigation into the June 13, 2012, CV-22B Osprey aircraft accident. The AFSOC commander convened an Accident Investigation Board to investigate the matter and designated Col. Hans Ruedi Kaspar, 23rd Air Force vice commander, AFSOC, Hurlburt Field, Fla. as the board president.

The Air Force CV-22 Osprey, assigned to the 1st Special Operations Wing, was flying in a two-ship formation when it crashed at approximately 6:45 p.m. during a routine training mission north of Navarre, Fla. on the Eglin Range.

All five 8th Special Operations Squadron aircrew members sustained various degrees of injuries when the tilt-rotor aircraft crashed, however none of the injuries were life-threatening.

The Accident Investigation Board President found by clear and convincing evidence that the cause of the mishap was the crew's failure to keep the aircraft clear of the lead aircraft's wake. This resulted in an 'uncommanded' roll to the left, rapid loss of altitude and impact with the terrain. The aircraft was destroyed upon impact with the loss valued at approximately $78 million.

The primary purpose of the board was to provide a publicly releasable report of the facts and circumstances surrounding the accident. The AIB report's full narrative can be found at www.afsoc.af.mil. An executive summary of the report will be available at http://usaf.aib.law.af.mil. The entire report is available by submitting a Freedom of Information Act request through the AFSOC FOIA Office, located at 100 Bartley Street, Suite 137E, Hurlburt Field, Fla. 32544-5273, (850) 884-2290, or to submit an electronic request, visit http://www.foia.af.mil.
